Job Responsibilities: The Adult Social Coordinator will be responsible for all facets of the operation of the program. Working with the Program Manager, responsibilities include: 1. Supervision of the Senior Direct Support Professionals and Direct Support Professionals in the performance of their job responsibilities including supervision of consumers, implementation of Day Hab Plans, training of staff regarding consumer specific needs and fire safety (including scheduled fire drills) and completion of all necessary documentation. 2. Maintaining staff ratios to ensure adequate supervision of consumers 3. Ensure all documentation is completed in a timely fashion and maintain individual record files. Documentation includes: Day Hab assessments, Day Hab plans, activities records, outcome data collection, monthly notes including distribution, DDPs (1 & 2), MAR to ensure all medications were administered, incident reports, seizure and behavioral records, necessary consents and all correspondence is documented. 4. Schedule all Day Hab annual and six month reviews. 5. Maintain communication with: Human Resources, IOSI, and AP/AR to ensure all business practices are followed according to policy. 6. Any additional responsibilities for the successful operation of the program.

Although Calverton is the main site, this position will require travel between Calverton and Riverhead with occasional visits to East Hampton/Southampon area.

Required experience and qualifications: Bachelor’s Degree in Human Service from an accredited college preferred plus two-three years related experience. Relevant experience may be considered in lieu of education.

Required Job skills: Successful candidate will possess strong interpersonal and communication skills, leadership qualities, and build relationships with consumers, care givers and with families. Practical skills include: excellent writing skills, strong organizational skills, flexibility and planning ability. Candidate will demonstrate an ability to grow with the program.

Hourly Rate: $24/hr.
