The Director of Social Services is an essential member of the Interdisciplinary Team of our long-term care nursing home facility with patients and residents who have complex requirements regarding their discharge or long term stay. This position, therefore, requires applicants to have social services work experience in a nursing home.

The Social Services Director is an integral part the Interdisciplinary leadership team that helps enhance the lives of our patients and residents each day. Highlights of your responsibility include:

• Resident assessments

• Care plans and behavior management programs

• Coordination of resident moves and room readiness

• Discharge Planning; acts as the facility liaison with contracted Hospice and Home Health Providers and area Agencies on Aging

• Facilitate and supervise provision of support groups to meet resident and family needs; assist with Resident Council as residents request

• Conduct quality assurance audits, prepare reports and attend month facility QAPI Committee meetings

• Assist in the education during new hire orientation and the annual whole house in servicing regarding Resident Rights, Abuse Prevention and the facility Social Media policy

• Other reasonable duties as assigned by the Facility Administrator

Required qualifications:

Bachelor’s degree in Social Work

• 4 years of Long Term Care social work experience

• Must be able to work well with elderly, emotionally upset and sometimes aggravated patients and residents

• Must have proven ability to establish a good working rapport with other staff, residents, families, community agency representative and the general public
