Precision Home Health, a home care agency with a main office located in Richmond, Virginia, is looking for a Registered Nurse (RN) with home care or home healthcare experience to service Roanoke County and the surrounding counties. Day to day will consist of assessment completion and submissions.

The Registered Nurse will assist with the development and ongoing completion of patients' assessments. In accordance with physician orders and the plan of care, the registered nurse may occasionally administer nursing care at the place of residence.

Responsibilities and Duties

  • provides services in accordance with the plan of care,
  • have at least one year of nursing experience,
  • performs an initial assessment or evaluation visit and regularly re-evaluates the patient’s nursing needs,
  • initiates the plan of care and necessary revisions,
  • prepares clinical and progress notes for each patient visit, as well as summaries of care conferences for his/her patients, in accordance with Agency policy.
  • informs personnel of changes in the condition and needs of the patient.
  • counsels the patient, family, and significant others on how to meet nursing and related needs.
  • processes order and notifies physicians of patient needs and changes in condition; completes certification and re-certification orders and discharge summaries.
  • determines the amount and type of nursing care needed by each individual patient.
  • supervises and teaches other nursing personnel.
  • completes and submits assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition in accordance with company-defined time frames.

Qualifications and Skills * A graduate of a recognized professional nursing school who is currently licensed in Virginia.

  • Two (2) years’ nursing experience is preferred.
  • ability to exercise initiative and independent judgment.
  • Reliable Transportation
  • Familiarity with MCOs and Medicare/Medicaid
  • Ability to build rapport with patients.
  • Strong problem solving and critical thinking skills

Job Types: Full-time, Part-time, Contract

Pay: $38.00 - $43.00 per hour
