Our Physical Therapists act as the patient’s partner throughout the trip of restoring motion and ensuring that the client will function at their personal best. Our PTs maximize the quality of life and movement potential within the spheres of promotion, prevention, diagnosis, treatment or intervention, and rehabilitation.

JOB SUMMARY:

As a Physical Therapist (PT) with A Care Connection Home Health you will:

• Complete OASIS documentation per Medicare guidelines (experience required)
• Provide therapy in the patient’s home including: exercises, stretching maneuvers, hands-on-therapy, and equipment to ease patients' pain and help them increase their mobility.
• Utilize a holistic approach in the provision of problem-specific therapeutic interventions and teaching/training activities in accordance with the plan of care
• Exceptional organizational and communication skills

QUALIFICATIONS:

• Area: Duval County and surrounding counties as needed
• Must have an active/clear therapist license in the state of Florida
• Must complete an Alzheimer's and Dementia Training Course
• Must be able to pass a Level 2 Background Screening
• Experience with Electronic Management Records
• Two years of therapist experience, preferred
• At least one year of home health experience/Oasis documentation, required
• Treatment of Lymphedema patients, preferred
