Job description
Successful Homecare Agency is looking Bi-Lingual (either Chinese or Korean Speaking) for Home Transition and Diversion Medicaid Waiver (NHTD) program Service Coordinators. This position will develop and write Service Plans for new participants, perform ongoing assessments of each client and develop a client specific Service Plan. Document all service issues and exceptions and communicate with staff and outside service providers.Ideal candidate will have strong advocacy skills, ability to multi task and handle paperwork. Also be able to effectively communicate both verbally and in writing and have the ability to make decisions and problem solve.
Local travel required throughout NYC.
Job Type: Part Time which can lead to a full time position.

Registered Nurse / Korean speaking or Chinese speaking
