This position is a second shift position with hours from 3pm. to 11pm. This position will be housed at the Eau Claire County Police Department.

The Crisis Social Worker performs duties related to, but not limited to case management of DHS 34 Crisis activities assigned by the Eau Claire County Crisis Supervisor as well as assisting the Eau Claire Police Department Co-Response Program with individuals in the city of Eau Claire experiencing mental health or substance use disorder concerns.

• This position engages in crisis intervention and refers those experiencing mental health issues, substance abuse, homelessness, and traumatic events to the appropriate resources and/or agency, including assisting patrol officers in emergency mental health decision-making, follow-up/safety planning, and maintaining and navigating on-going relationships with high-utilizing community members. Provide on-scene assessments, offer recommendations on services, and follow-up contact, while documenting all activities per Eau Claire County policy.
• Work in connection with law enforcement and the Co-Response Coordinator to determine the appropriate crisis response.
• Identifies, through case management and/or access services, individual needs which include medical, psychological, vocational, social, familial, and daily living spheres
• Explains the consequences of non-compliance with court orders to adults and families who are involuntarily referred, monitors utilization of services, and informs the court of adherence to court orders.
• Facilitate adult and families' access to services, monitors cases to ensure that services are provided in an appropriate and timely manner, reviews progress, and terminates services when stated goals are reached.
• Educates and advises individuals and families about available community resources and how to independently gain access to needed services.
• Works to achieve services and treatment objectives without duplication of effort and with maximum effective use of staff time and available resources.
• Maintains a complete record for all assigned cases; completes required written assessments, reports, and other related paperwork as necessary.
• Documents contacts and services, maintains files and data for reporting purposes, prepares court reports, collateral dictation, and prepares other records and reports.
• Attends and participates in staff meetings, supervision times, and other regularly scheduled meetings; develops professional knowledge and skills by attending training events, conferences, and workshops.
• Serves as advocate for clients within the community.
• Speaks to the community and other organizations to provide public education.
• Performs other duties as assigned.

• Bachelor's Degree in Social Work or a related field; and,
• Minimum of 6 months of paid or unpaid experience in family, children, or adult case management services; or successful completion of an internship in the specific area of hire
• Must have DHS 34 Certification or obtain within three months of accepting position
• Must have access to private transportation for work-related duties

• Must be in compliance with the State of Wisconsin Social Worker Certification requirements as a condition of employment by meeting one of the following requirements:
  • Certified as a Social Worker in Wisconsin
  • Possess or be able to obtain a temporary certificate within 1- year of hire and become certified within the specified time limits of the temporary certificate
• Appointment will be conditional upon successful completion of criminal and caregiver background checks
• Possess a valid Wisconsin Driver’s license.
