We are now hiring a Full-time Community Navigator to join our Merrionette Park team!
ESSENTIAL JOB FUNCTIONS:
- Provide navigation across sectors and providers to ensure coordination and service access/completion for R3 participants
- Complete the screening tool to identify service/resource needs;
- Make recommendations for service/resources, garner interest, and complete an intake form of basic client information (i.e., contact information-best way to engage (texting, emailing, calling);
- Complete a client consent form to support care coordination internally through MFS and externally with partner networks;
- Provide as needed accompaniment, assistance with paperwork, access to support services (i.e., transportation stipends)
- Complete follow-up with both the client and referral entity to ensure the completion of services.
- Provide information and connection for applying for public benefits
- Work collaboratively with Program Coordinator to provide outreach and expand resident knowledge of available resources/services through R3
- Complete community canvassing which markets the program and present contact information to initialize a screening and intake to service.
- Work collaboratively with the Blue Island Robbins Neighborhood Network (BIRNN) to establish a formalized referral process to engage clients across partner agencies.
QUALIFICATIONS:
- Bachelor’s Degree in Human Services or equivalent from an accredited university or college required.
- 3 years of relevant experience in the social service field required.
- Strong interpersonal skills.
- Good judgment in maintaining responsibilities of the position in advocating for the best interest of clients served.
- Must have access to a vehicle, proof of insurance and valid driver’s license to perform essential job functions required.
- Must have basic computer skills.
- Strong oral and written communication skills.
- Ability to work effectively with diverse individuals and groups.
- Resident of one of the targeted communities strongly preferred (Blue Island, Robbins, Posen, Dolton, Harvey, Ford Heights)
ADDITIONAL REQUIREMENTS:
- MFS staff are required to provide proof of vaccination or have an approved Medical exemption prior to starting employment. If an approved Medical is obtained, weekly testing is required.
