Job description
BACHELOR'S LEVEL: Family Liaison needed to provide services to at-risk youth and families in our Wraparound Program based in Hartford, MI. The Liaison completes strengths-based assessments & develops a comprehensive child and family treatment plan based on identified strengths and family support systems. Services are provided in home and community settings.
DUTIES INCLUDE:
Conducting strengths and psychosocial assessments.
Developing family service plans with the Child and Family Team.
Providing or arranging outreach crisis intervention & case management services.
Linking and advocating with other agencies to ensure children and families receive needed health and human services.
Providing support that enhances or restores the capacity for social functioning in the home, school, and/or community.
REQUIRES:
Bachelor's degree in mental health field.
Two years of relevant experience, including experience with SED youth.
Understanding of criminal justice delinquency, neglect/abuse issues, and organizational and family systems and/or social work practices as it relates to family systems.
Familiarity with resources & services in Van Buren County.
Position is full-time (37.5 hours per week).
