General Scope of Responsibilities

The WISE Facilitator will be responsible for the coordination of the WISE and SBIRT services under the guidance of the WISE Coordinator. These responsibilities will include scheduling WISE classes, implementation and facilitation of the WISE/SBIRT pilot program, maintenance of program records for reporting, outreach and other duties as assigned. The WISE Facilitator will report to the WISE Coordinator.

Qualifications:

• A bachelor’s degree in human services, education, social work, public, and/or geriatrics is preferred, in addition to
• Two years’ experience in the field.
• Credentialed Prevention Professional or eligibility desired.
• Strong presentation, organizational and verbal and written communication skills and a working knowledge of Microsoft software, required.
• Valid NYS Driver’s License and access to personal transportation during working hours is required.

Hiring is contingent upon clearance of Catholic Charities and program funders required background checks sanction screenings and Central Registry Clearance plus completion of mandated trainings.

Job Duties and Responsibilities

• Attend required trainings to conduct the WISE/SBIRT pilot program.
• Assist the WISE Coordinator on the implementation of WISE/SBIRT pilot program.
• Development and coordination of WISE/SBIRT pilot program schedule.
• Maintenance of WISE Program documents including: WISE program handbook, training materials, SBIRT screening tools, intake forms, media consent forms and other documents as required.
• Facilitate WISE classes, including preparing class materials, ordering refreshments and coordinating with host site location.
• Maintenance of records to meet program and funding reporting requirements.
• Assist with marketing for WISE/SBIRT pilot program and attend outreach events.
• Assist WISE Coordinator to meet program goals and requirements.
• Obtain and retain knowledge of alcohol and other substance misuse/abuse and its impact of the individual and family system.
• Provide substance abuse screening, brief intervention, and referral to treatment (SBIRT) services utilizing evidence-based practices and programs as defined by NYS OASAS.
• Participate in agency and program staff meetings and supervision sessions on a monthly basis.
• Coordinate with other programs and Catholic Charities staff in the delivery of comprehensive services.
