POSITION SUMMARY:

The WIC Nutritionist for the WIC Program is a professional who supports the mission, goals and objectives of the North Country Family Health Center, Inc. by improving nutritional services in the WIC Program. This staff person will follow standards for the assessment process used to determine WIC eligibility and personalize nutrition education, high risk care plans, referrals and food package tailoring.

QUALIFICATIONS:

  • (Preferred) Registered Dietitian Nutritionist (RD/RDN), or
  • Bachelors, Masters, or PhD in Nutrition, or
  • CPA+ (CPA3 grandfathered in under last revision of Policy #1460 in 2013), and
  • Valid driver’s license
  • Excellent customer service and communication skills
  • Ability to travel among Jefferson and Lewis Counties, and for trainings
  • Flexibility in work hours which includes working evenings and Saturdays

ORGANIZATIONAL RESPONSIBILITIES AND STANDARDS

  • Completes annual mandates and health assessments.
  • Recognizes potential problems and corrects or reports to supervisor immediately.
  • Abides by all Department of Health and agency policies.

TASK III. PARTICIPATES AND SUPPORTS PERFORMANCE IMPROVEMENT ACTIVITIES.

PERFORMANCE STANDARDS:

  • Actively serves on QI teams when requested.
  • Assists in QI process by cooperating in surveys, data collection and other activities.
  • Actively seeks opportunities for process improvement and initiates as appropriate.

TASK IV. USES APPROPRIATE INTERPERSONAL SKILLS.

PERFORMANCE STANDARDS:

  • Cooperates with fellow staff to accomplish tasks.
  • Demonstrates respectful manner toward patients, visitors, staff and members of the community.
  • Demonstrates a willingness to help when needed for additional tasks.

TASK V. DEMONSTRATES EFFECTIVE AND PRODUCTIVE PERSONAL BEHAVIORS.

PERFORMANCE STANDARDS:

  • Uses time in an efficient manner, completing duties timely and safely.
  • Complies with agency time and attendance requirements.
  • Is always courteous and professional to other staff, patients and members of the community.
  • Maintains confidentiality in full compliance with the letter and spirit of NCFHC and WIC Policies. Must sign Confidentiality/Conditions of Employment Statement.
  • Demonstrates initiative for personal growth by attending in-services and meetings.

POSITION SPECIFIC RESPONSIBILITIES AND STANDARDS

TASK I. ADMINISTRATIVE/CLINIC DUTIES

PERFORMANCE STANDARDS:

  • Be cross-trained to cover all areas of clinic to avoid a gap in services and maintain clinic flow (ex. intake, height, weight, hemoglobin, answering phones, auto-dialer).
  • Perform tasks to ensure participants are contacted with reminder calls, rescheduling no-show appointments and/or discharged according to policy.
  • Assigned responsibilities, such as: Breastfeeding Coordinator, Voter Registration Coordinator, Outreach Coordinator, Substance Abuse Coordinator, Workplace Committee, Safety Committee, van maintenance, clinic supervision, ordering supplies, and answering phones.

TASK II. COMPLY WITH PROGRAM AND AGENCY POLICIES AND PROCEDURES.

PERFORMANCE STANDARDS:

  • WIC Nutritionist will assess and counsel/educate any participants with basic nutritional needs, but will focus on participants whose nutrition or health concerns require an advanced level of nutrition expertise (high risk).
  • Conduct a complete nutrition assessment to determine nutritional risk, program eligibility and enroll participants.
  • Develop and implement high risk care plans for all high risk participants.
  • Provide oversight to CPAs in their follow-up of high risk participants.
  • Review and approve medical documentation for participants with Food Package III.
  • Prescribe, tailor, authorize, return and reissue all food packages, including Food Package III.
  • Promote and provide education on the Farmers Market Nutrition Program

TASK III. CORE SERVICES

PERFORMANCE STANDARDS:

  • Provide leadership in the planning, conducting, and evaluation of participant-centered nutrition education (individual/family sessions or facilitated group discussions) responsive to the identified needs/interests of participants.
  • WIC Nutritionist will facilitate groups as needed.
  • Promote healthy lifestyles plan, including physical activity options in the clinic and community.
  • Support participants in their decision to breastfeed, refer to Enhanced Peer Counselor Program and assist in providing a breastfeeding-friendly environment.
  • Perform hematology testing and anthropometry as needed.
  • Responsible for promoting extended hours, including lunch time appointments, in order to increase access to working participants.

TASK IV. DOCUMENTATION

PERFORMANCE STANDARDS:

  • Document any participant-centered nutrition education (individual/family sessions or facilitated group discussions) responsive to the identified needs/interests of participants, especially those participants whose nutrition or health concerns require an advanced level of nutrition expertise (high risk).
  • Provide and document appropriate follow-up referrals in NYWIC to other health and social service agencies for participants, especially participants whose nutrition or health concerns require an advanced level of nutrition expertise (high risk).
  • Implement optional and required individual care plans for participants, especially participants whose nutrition or health concerns require an advanced level of nutrition expertise (high risk).
  • Track participants’ progress, including goals set to improve health and document outcomes, especially participants whose additional nutrition or health concerns require an advanced level of nutrition expertise (high risk).
  • Indicate the requirements for participant’s next appointment (ex. high-risk counseling, immunizations, height, weight, hemoglobin) in participant’s record.

TASK V. TRAININGS AND QUALITY ASSURANCE

PERFORMANCE STANDARDS:

  • Attend and participate in all mandated trainings.
  • Obtain 24 hours of continuing education every 3 years in topics relevant to WIC.
  • Keep documentation of all trainings up to date per Local Agency Policy.
  • Provide leadership in developing and conducting nutrition training of LA staff in nutrition education topics.
  • Perform quality assurance as instructed by WIC Director.
  • Review active and discharge charts per QA policy.
  • Provide leadership in orienting/training/mentoring CPAs and nutrition support staff.

TASK VI. OUTREACH

PERFORMANCE STANDARDS:

  • Perform Public Health Detailing as directed by WIC Director.
  • Participate in outreach activities to expand caseload and agency visibility.

TASK VII. COMMUNICATIONS

PERFORMANCE STANDARDS:

  • Responsible for reviewing all NYS DOH memos/communiqués/policies available in NYWIC.
  • Responsible for accessing NCFHC emails and responding accordingly.
  • Maintain NCFHC updates via agency newsletter and memos.
  • Responsible for all information provided at staff meeting (if unable to attend, obtain from co-worker or meeting minutes).
  • Keep open communication with co-workers, WIC Director, and Site Lead to improve work flow on a daily basis.

TASK VIII. REMOTE SITE CLINIC RESPONSIBILITIES

PERFORMANCE STANDARDS:

  • Prepare equipment and supply boxes for clinic.
  • Safe lifting and carrying of heavy equipment.
  • Assist in the transportation of staff and supplies to WIC clinics.
  • Set-up and tear-down clinics.

TASK IX. RESPONSIBLE FOR NEATNESS OF OFFICE AND CLINIC AREAS.

TASK X. PERFORM RELATED DUTIES AS ASSIGNED.

Job Type: Full-time

Pay: $23.27 per hour
