TITLE: VDC Case Manager/Caregiver Program Coordinator

AGENCY: Partnership for Public Health

REPORTS TO: ServiceLink Program Manager

INTRODUCTION:

ServiceLink Resource Center is a program of the NH Department of Health and Human Services. Through a contract Partnership for Public Health, ServiceLink Resource Center helps individuals access and make connections to long term services and supports, access family caregiver information and supports, explore options and understand and access Medicare and Medicaid. ServiceLink is designated as New Hampshire's Aging and Disability Resource Center and the NHCarePath. Possible hybrid after training period.

RESPONSIBILITIES: VDC Case Manager

The VDC program gives Veterans of all ages the opportunity to receive the home and community based services they need in a consumer-directed way. As part of this program, Veterans and their caregiver have more access, choice and control over their long term care services.

The CSR VDC Case manager assists veterans in this program to remain in their home or in their community.

1. Develop and maintain and understanding of Partnership for Public Health & ServiceLink Resource Center programs, services, and functions across subject matter domains to facilitate efficient access to services and supports;

2. Provide participant directed and person-centered case management to a caseload of Veterans in partnership with the VA Medical Center program director, serve as a point of contact for the Veterans on the caseload;

3. Works with ServiceLink team, community members, and local providers in catchment area to identify veterans to build an enhanced knowledge of services and supports available, provide education, and offer support;

4. Works with Veterans to do assessments, create plans of care, develop budgets, and assist in implementing services that may be made available through Veteran-Directed Care;

RESPONSIBILITIES: Caregiver Coordinator

The Caregiver Coordinator provides assistance to family members and others who provide day to day care for another individual without compensation. Services include information, counseling, education and funding for eligible respite care, and are coordinated in partnership with the NH Bureau of Elderly and Adult Services, NH Family Caregiver Support Program (NHFCSP) through ServiceLink Aging and Disability Resource Centers located throughout New Hampshire.

Those served through the program include family caregivers of frail older adults, younger adults with disabilities, grandparents raising their grandchildren, and caregivers of people diagnosed with Alzheimer's disease and dementia.

The services provided to NH caregivers through NHFCSP are numerous and include assisting individuals to continue to live in their homes and communities longer avoiding nursing home placement.

1. Provides consumers person-centered assistance in understanding and accessing services under, the NH Family Caregiver Support Program (NHFCSP);

2. Works with caregivers to do assessments, create plans of care, develop budgets, and assist in implementing services that may be made available through NHFCSP and other community support services;

4. Works with the fiscal management agency (currently Gateways Community Services) to ensure that payment for services occurs in a timely and appropriate fashion;

5. Participate as part of the ServiceLink team to provide information, referrals, assistance, and follow up to clients through home and community visits, telephone, walk-in, group sessions, or email as needed/appropriate;

6. Assist ServiceLink Program Manager in completing all required reports and coordinating record keeping for NHFSCP as required;

7. Assists ServiceLink Program Manager and Bureau of Elderly and Adult Services on statewide initiatives relative to the NHFCSP;

8. Develop, implement and conduct outreach and public education sessions as needed;

9. Attends staff meetings and trainings as required;

10. Performs other duties as assigned by Partnership for Public Health Executive Director, Director of Human Service Programs, or designee, that will assist with ServiceLink team functions to include serving on various committees, as requested.

EDUCATION, EXPERIENCE, AND SKILLS REQUIRED:

1. Bachelor’s degree preferred or Associate’s degree with equivalent experience.

2. Obtain certification as Alliance of Information Referral Specialist (AIRS) and Person Centered Options Counseling;

3. Experience in case management, social work, or other related human services field;

4. Experience working with older adults, family caregivers, and/or persons with disabilities,

5. Working knowledge of local, state, national resources and human service delivery system;

6. Demonstrated skills in: listening, customer service, interviewing, and understanding of services, advocacy, and documentation;

7. Good interpersonal skills, openness and flexibility in working with diverse groups, and enthusiasm for working collaboratively with a team;

8. Good computer skills;

9. Ability to establish and maintain effective working relationships with groups such as: representatives of other social service agencies, institution officials, the public, and clients;

10. Ability to organize and present at educational and outreach events;

This position requires:

  • Minimal requirement to reach at or above shoulder level; occasional reaching below shoulder level required.
  • Ability to travel throughout the territory required.
  • Ability to work a flexible schedule, which may include night and weekends.
  • Ability to tolerate prolonged sitting and/or standing and to lift up to approximately 25 pounds to waist high level whenever necessary.
  • Mobility includes regular sitting, some standing and walking.
  • Maintain a valid driver’s license, good driving record and access to a fully insured car;
  • Criminal Records and Bureau of Elderly and Adult Services Registry record check and acceptance by agency.
