Our goal is to promote the mental health of all New Yorkers, with a particular focus on providing hope and recovery for adults with serious mental illness and children with serious emotional disturbances.

• Serves as a member of a multidisciplinary treatment team; presents clinical information and observations, and coordinates care and treatment planning for individuals.

• Provides a variety of social work and treatment services to individuals and families that include life skills, vocational and career training, and community integration that assists individuals in achieving maximum participation in work and social activities.

• Determines discharge service needs and appropriate community resources; and follows up with community providers to ensure individuals are successfully integrated into the community.

• Prepares and maintains documentation and records, including progress notes, observations, recommendations, and discharge plans

Job Type: Full-time

Pay: $61,494.00 - $65,916.00 per year
