The successful candidate for this position will have:
- a Bachelor's or Associate's Degree in Social Work, Arts, or Liberal Arts;
- experience in long-term care or case management discharge planning;
- excellent work history;
- commitment to team-oriented outcomes and care; and,
- excellent communication and leadership skills.
