Job description
Patient: Fun 6 year old little girl needs a nurse to go to school with her ASAP!!!!
Medical :Type 1 Diabetic diagnosed at 18months. She also has a h/o right club foot, and occasionally wears a brace on this foot. She is very active, but is at risk for increased falling/or tripping.
Location : South Valley Elementary School
Start date & Hours : 12/5/22 , andwe are looking or a nurse to cover during this time. School hours would be 7:45am 3:45 pm Monday - Friday.
Transport on bus is involved for the am, Nurse reports to school and ride bus to patient home and back to school. Afternoon transportation is provided by another nurse.
Nursing duties include: Continuous Blood glucoses monitoring via DEXCOM and a Omnipod Insulin pump, pre treating pt with insulin according to anticipated carb counts before snackand lunch. She has a cell phone which is connected to the DEXCOM so the nurse sees her blood glucose readings continuously. There will also be a back-up manual blood glucose monitor available for blood sugar readings PRN.
