Position: RN Case Manager
Community Clinic is seeking a full-time RN Case Manager to work directly with its multi-disciplinary clinical teams. This position provides ongoing case management support and expertise through comprehensive assessment, planning, implementation and evaluation of individual patient needs to improve outcomes and overall health. A current RN license is required. A minimum of 2 years of utilization review/case management experience is preferred. Some local travel. Excellent benefits package including: insurance, 403(b), PTO and paid holidays.
