Under the leadership of the Regional Operations Lead, and in collaboration with the Regional Clinical Lead, the Network Clinical Team and the Network Performance Team, the Regional Performance Manager and Practice Liaison leads quality initiatives and projects that drive improvement in quality and efficiency metrics at the practice level. This person has the responsibility to review performance data provided by the Network Performance team and utilize practice operations expertise to develop initiatives that will lead to critical improvements in performance and provider, practice staff and patient experience. This role will work closely with Regional Operations and Network Performance Leadership to direct resources working on performance improvement and patient, clinician and practice staff experience in the Region.

Candidate must be willing to travel to practices located in the Greater Lowell area

Responsibilities

  • Works as a point of contact in the region for performance in value-based agreements and to assist in overseeing and coordinating Quality and other performance management resources in the region
  • Work closely with Network leadership to identify new opportunities from existing practices / customers and to solve any issues that arrive with current clients.
  • Responsible for projects initiated by Regional Lead to develop the scope, bring together stakeholders and develop plan to implement in practices.
  • Functions as an expert in HEDIS and other quality and efficiency performance measures.
  • Provide guidance and monitor adherence to Network policies.
  • Work in tandem with the PQAs assigned to shared practices and work together to provide a coordinated and seamless customer experience
  • Participate in project teams at the Network and Region level as needed and complete assigned work as directed.
  • Escalates problems and physician practice concerns to leadership as needed.
  • May present at Region meetings
  • Responsible for the development and implementation of patient experience improvement programs.
  • Participates in development of training and educational programs to assure successful implementation of Quality Improvement initiatives and as needed, visits practices to implement programs.
  • Maintains collaborative team relationships with peers and colleagues to effectively contribute to the working group’s achievement of goals, and to help foster a positive work environment.
  • Performs other similar and related duties as required or directed.

Qualifications

  • Associate or Bachelor’s degree preferred
  • Minimum of 5 years’ experience working in a practice and familiar with practice office workflows
  • Demonstrated ability to interact successfully with clients utilizing excellent communication and customer service skills
  • Knowledge of ambulatory and physician practice roles, processes, and workflows
  • Demonstrate ability to use applicable EMR and Population Health Systems
  • Ability to navigate difficult/demanding personalities while maintaining a strong professional relationship
  • Strong customer relationship building skills
  • Ability to collaborate in a multidisciplinary environment
  • Self-driven and able to handle multiple projects simultaneously
  • Ability to work and communicate with physicians
  • Excellent written and verbal communication skills, including listening skills, are critical. The individual must be comfortable presenting to groups, both large and small, and individuals, presenting ideas professionally and clearly and focusing discussions on the appropriate issues.
  • Excellent project management skills. Demonstrated ability to develop, implement, and manage results-oriented programs that achieve the targeted goal.
  • Excellent organizational skills, and good judgment; must be a team player, positive, flexible, and creative to ensure the improvement strategy and plans are successful. Able to supervise team members.
  • Solid knowledge of healthcare plans and implications to clinical programs.
  • Possess flexibility and the ability to work independently
  • Excellent creative thinking and problem solving skills.
  • Data driven, possessing solid knowledge and experience with developing and analyzing quality and efficiency reports, along with total quality improvement strategies, approaches, techniques, tools and analytical methods.
  • Working knowledge of HIPAA and other appropriate legal and regulatory requirements which would impact work.
  • Proficiency in Microsoft Office applications, including Excel, PowerPoint and Word.
