Exciting opportunity to provide new models of care for walk-in/urgent care access including days, evenings and weekends. The new CHA CBHC Cambridge program provides behavioral health evaluation and initial treatment services for individuals of all ages.


Under the general supervision of the CBHC urgent care/walk-in service and CBHC clinical staff, provides clinically-based peer Recovery Support Services to:
Support patient engagement into Recovery Support and addiction treatment
Collaborate with other members of the care team to identify needs and resources with the patient, family and health care team, create a treatment plan appropriate to the patient’s needs and resources.


Responsibilities:
  • Partners with patients seeking recovery, based on their needs in recovery-oriented skills, activities of daily living, socialization, and community integration; working to get them ‘plugged in’ with a variety of services that might enhance their overall recovery.
  • Collaborates with CHA staff to improve engagement of patients in ongoing recovery support and treatment after discharge from ED, hospital, residential program, or detox - and help patients connect with CHA-based outpatient treatment for ongoing care - such OBOT nurses and services, as well as the Outpatient Addictions Service at CHA.
  • Engages patients via telephone, on site at CHA, and in the community to optimize and coordinate their care
  • Engages and helps navigate patients to community-based resources
  • Assists the clinical team in the development of an individual action plan, or supports the person served in developing a wellness plan for themselves.
  • Coaches ‘recoverees’ who have developed their own wellness plan to review their goals and objectives on a consistent basis.
  • When appropriate, assist people in identifying and accessing sponsorship.
  • Develops and enhances peer-driven educational services.
  • Continually educates colleagues on core principles and roles of recovery support and harm reduction.
  • Documents patient contacts in the EMR in accordance with established methods and procedures.
  • Facilitates groups to provide support for increased stability through recovery, a better understanding of symptoms of relapse, as well as symptoms of mental illness.
  • Provide ongoing assessment, problem solving, side-by-side services, skill training, supervision and environmental adaptations to assist patients with activities of daily living (ie. Identifying and attending self-help meetings, support with sponsorship)


Qualifications:

Minimum 10 years’ sobriety and 5-10 years’ experience in peer recovery support work
Experience working as a Recovery Coach in a hospital/Emergency Department setting
Minimum High School diploma or equivalent, Bachelor’s degree desirable
Certification:
Current Addiction Recovery Coach certification in the Commonwealth of Massachusetts
Valid Driver’s License in the Commonwealth of Massachusetts
