Job Duties List Quality Life Advocate (QLA)

Qualifications:

1. Must be an experienced CNA

2. Must have experience with both behavioral and geriatric units

3. Must maintain CPR certification, at facility's cost

4. Must be CALM certified - training that the facility provides

JOB DUTIES

1. Will be responsible for orientation for all CNA’s/NA’s for assigned unit and completing the orientation checklist.

2. Will be responsible for covering staff call ins/ shortages

3. Will be on call for assigned shift

4. Will monitor all CNA documentation:

o ADL Charting

o Inventory sheet initial, updates, discharge

§ SNU-Silverware Count Log

§ SNU-Environmental Rounds

§ SNU-Face checks

§ SNU-1:1 checks

§ SNU-Behavioral Monitoring Log (72 hours)

o Meal audit completion (no holes)

o Communication Book entry

5. All CNA’s and NAs will report to the QLA who will report to the RCC or charge nurse any issues or concerns with residents and or their condition.

6. SNU Assure environmental rounds are completed.

7. Assure all cleaning duties are completed per schedule and perform organizing of clean, dirty utility, snack rooms, etc.

8. Check that all showers/ baths were completed. Report to charge nurse or RCC those not completed or refused.

9. Assure snacks are passed and documented on ADL sheet

10. SNU-check cigarettes, snacks and re-order as necessary.

11. CQI meetings on areas in need to improvement.

12. SNU-conduct Reflection Group each evening M-F

13. Participate in Interview Team for new staff

14. Coordinate all routine daily activities for residents with CNA’s and NA’s

15. Leave voice mail report each shift

16. Follow up on all special needs equipment of residents (eating utensils, walkers, etc)

Job Type: Full-time

Pay: $15.00 - $17.00 per hour
