What we are looking for:

• 1 year of working as a telehealth registered nurse conducting phone visits
• Fluency in English and Spanish
• A positive attitude: you are a team player, and you work effectively with different teams.
• Ability to work independently with minimal supervision.
• Excellent interpersonal and communication skills.
• Proven organizational skills with high attention to detail and the ability to prioritize.
• Proficient in Microsoft office and Excel

General Responsibilities:

• Perform remote telehealth evaluations of individuals seeking long term care services and support
• Communicate timely and effectively with all members of the patient care team, as well as relate courteously and appropriately to all internal and external customers
• Provides patient/aide education on chronic and acute illness management, medications, and symptoms to report
• Documents and maintains records of telephonic interactions with patients and their health care teams.
• Improves overall clinical communication and quality of care.
