  • Must be available on-call to support staff, find coverage or cover shifts as needed
  • Supervise assigned Direct Support Professionals, to ensure employees are completing all job duties while on shift

Program Coordinator Job Description

Responsible to:

The Program Coordinator answers directly to the Program Director.

Work Hours:

Monday through Friday. Hours vary depending on participant and staff needs. May involve evening hours and weekend hours as needed. May include services delivered in the community as well as in the participant’s home. Will include rotating on call every other week.

Specific Duties:

  • Responsible and accountable for implementing the policies and procedures approved by the governing authority.
  • Implement written administrative policies and procedures which comply with applicable rules.
  • Implement policies and procedures for employees and residential habilitation provider training, quality assurance, evaluation, and supervision.
  • Conduct regular, at least monthly, employee and residential habilitation provider meetings to review program and general participant needs and plan appropriate strategies for meeting those needs.
  • Assist in making all records available to the Department for review or audit.
  • Implement all policies addressing safety measures to protect participants, employee, and residential habilitation providers as mandated by state and federal rules.
  • Ensure that agency personnel, and providers including those providing services under arrangement, practice within the bounds set forth by the applicable state licensure boards.
  • Assist to implement policy decisions concerning all services.
  • Assist in providing a continuing and annual program of overall agency evaluations
  • Help assure that appropriate training, space requirements, support services, and equipment for staff or residential habilitation providers are provided to carry out assigned responsibilities
  • Cooperate in participating in a system by which to coordinate with other service providers’ continuity of the delivery of residential habilitation services in the plan of service.
  • Attend Professional meetings as needed.
  • Provide all skills training for direct staff.
  • Maintain weekly contact with TSC’s and Guardians.
  • Review and follow up on incident reports.
  • Monitor scheduling needs.
  • Conduct weekly meetings with houses.
  • Complete quarterly on-site reviews.
  • Complete Provider Status Reviews
  • Other duties as assigned.

Special Conditions:

Position requires that employee/contractor possess a safe driving record and an Idaho Driver’s License. Must be able to lift participants as needed, physically assist in activities of daily living, pull and push a wheelchair, get in and out of the homes of participants. Must be able to make observations of non-verbal behavior and check tasks and activities for completeness. Perform all activities of daily living that are taught. Position requires completion of a criminal history and background check and a subsequent approval from Health & Welfare. This process must be initiated prior to providing services.

Qualifications:

  • Is at least twenty-one (21) years of age
  • Has satisfactorily completed a criminal history check in accordance with IDAPA 16.05.06
  • Has a minimum of three (3) years of experience in service delivery to persons with developmental disabilities with at least one (1) having been in an administrative role.
  • Has at least one year of experience working directly with persons with developmental disabilities;
  • Meets QIDP qualifications in Federal Code 42 CFR 483.430
  • Must be CPR and First Aid certified or available to attain certification.
  • Be free from communicable diseases
  • SAMS Certification in order to assist with medication

Job Type: Full-time

Pay: $20.00 - $22.00 per hour
