The BHC will serve as a core member of the interdisciplinary Patient-Centered Primary Care Team, in the Primary Care Behavioral Health Service at Aviva Health to improve patients’ medical outcomes.

ESSENTIAL FUNCTIONS:

  • Serves on the multidisciplinary primary care team providing primary care behavioral health services.
  • Conducts warm hand-off assessments.
  • Utilizes excellent clinical judgment in assessing patients’ mental/ behavioral health condition(s) or immediate needs.
  • Applies high-quality, appropriate brief, short-term treatment interventions based on clinical diagnosis and assessment of the patient’s presenting problem(s).

QUALIFICATIONS:

  • Minimum of master’s degree in Clinical Social Work (MSW) or Master’s degree in Professional Counseling or Marriage and Family Therapy. Or, Ph.D. or Psy.D. in clinical or counseling psychology.
  • Current and active licensure in the State of Oregon or license eligible within 6 months of hire.
  • Or, CSWA or Registered Associate in Professional Counseling or Marriage and Family Therapy, working towards Oregon licensure as an LCSW, LPC, or LMFT; or Psychologist Associate; or QMHP certification.
  • Passion for serving the underserved.

Job Type: Full-time

Salary: $74,040.00 - $92,550.00 per year
