We're looking for someone to join our team as a Population Management Associate (Outpatient) who:

Coordinates and administers population management activities within assigned panels of outpatients with a focus on providing patient-centered, quality healthcare across a population. Identifies, tracks and contacts patients for the purposes of achieving program goals and achievements associated with improving the quality of care. Maintains records, statistics and data bases associated with population management program goals and regulatory requirements. Leverages aggregate patient data across multiple health information technology resources and generates reports for analysis.

Essential Functions:

  • Corresponds with patients to support their efforts to manage their own health by following up with patient compliance with medication use, physician visits and reinforcing self-care management skills. Uses disease registry reports to identify and contact patients who routinely fall through the gaps and need follow up care with their primary care physician (PCP).
  • Gathers and organizes patient health care information, records data, maintains population management data bases relating to specific care management programs. Generates utilization, compliance and other reports associated with population care management and program performance. Collects and organizes clinical, financial and operational data together from across population management databases and disease registries to provide actionable analytics for providers to improve efficiency and patient care.
  • Uses and runs panel management and disease registry reports to identify subpopulations of patients who might benefit from additional services. Identifies groups which may include: patients needing reminders for preventive care or tests, patients overdue for care or not meeting management goals, patients who have failed to receive follow-up after being sent reminders, and patients who might benefit from discussion of risk reduction.
  • Performs other job-related duties as assigned.

You'll be successful with the following qualifications:

Education:

  • High School Education/GED or equivalent: Preferred
  • Associate's/Technical Degree or equivalent combination of education/related experience: Preferred
