Job Summary
The Population Health Coordinator will be responsible for coordination of population health initiatives and providing patient care under supervision of a Clinical Nurse/physician. The incumbent will collaborate within the Hospital’s provider network to improve quality, efficiency, and cost of healthcare services on behalf of its patient population
Education Requirements:
Incumbent must be a graduate of a licensed practical nurse program.
Experience Requirements:
Effective interpersonal communication skills in order to effectively collaborate with physicians and instruct patients and their families. Three years of relevant management experience with knowledge of: Population health programs, including annual wellness visits, chronic care management, and similar programs Accountable care organizations (“ACO”) and impact of Medicare Shared Savings Programs Quality payment programs from various other third-party payers
Certifications/ Licensures Requirements:
Licensed Practical Nurse
Physical Requirements:
Light Work: Frequent lifting and/or carrying of objects weighing up to 10 lbs. with a maximum lifting of 25 lbs.
