This position is part of our Interprofessional, Collaborative Alignment Resulting in Exceptional Patient Care (ICARE PC) team. As an ICARE PC pharmacist at CHI Health Lakeside, you have a unique role providing bedside pharmacy care to our patients. You will work alongside our doctors, nurses, occupational therapists, and nursing assistants in our Progressive Care Unit to safely prescribe, dispense, and administer medications to our patients.

To excel in this role, you must enjoy learning new concepts and researching information in real-time to provide solutions for real people. You should also continually look for ways to go above and beyond for your patients to deliver them an exceptional experience along with exceptional care.

We provide a full range of services including: maternity, cancer, surgery, diagnostics, and emergency.

Responsibilities

  • Work with nurses, nursing assistants, and occupational therapists to provide patient bedside care, including medication administration and education.
  • Detailed involvement in transitions of care for patients from admission to discharge, including obtaining medication histories, completing medication discharge clinical assessment, and providing discharge medication education.
  • Complete patient profile review by reviewing patient’s allergies, weight, renal function, lab results, etc. Make recommendations for drug, route, or dose optimizations when appropriate.
  • Perform a variety of pharmacy consults, such as anticoagulation dosing and monitoring, renal dose adjustments, and antimicrobial selection and dosing.
  • Attend interdisciplinary meetings to provide pharmacy policy information and/or act as a drug resource.
  • Review medication orders for safety and accuracy.
  • Provide CommonSpirit’s level of exceptional service to all patients.

Qualifications
  • BS or Doctor of Pharmacy degree
  • Pharmacist License
