HVCS a Division of Cornerstone Family Healthcare is actively recruiting for a Nutrition Coordinator to join our growing team in Hawthorne, NY.

RATE OF PAY/SALARY: $21.98 per hour

WORK LOCATION(S): Hawthorne, NY

STATUS: Full Time

FLSA Status: Non-exempt

JOB SUMMARY:

  • Under the supervision of the Program Supervisor, and the direction of the Program Supervisor for the THRIVES South Nutrition Program, the Nutrition Coordinator is responsible for assisting in the coordination of THRIVES Food Pantries, as well as coordination of care and services in Putnam and Westchester counties.
  • Arrange for clients to receive food bags either by home delivery or by appointment at their designated THRIVES pantry.
  • Conduct program intakes, screenings, and facilitate client enrollment, including completion of data entry into program database, and arrange for required assessments and reassessments with contracted Registered Dietitians in accordance with program requirements.
  • Manage food inventory for all THRIVES food pantries in order to provide nutritious and healthy food for clients, including the coordination of all aspects of food ordering and deliveries, as well as the preparation of client food bags for pick up during pre-arranged appointment times.
  • Monitor equipment and space used to store THRIVES food for distribution to clients, to ensure adherence to food safety best practices.
  • Establish and maintain relationships with client providers to perform case conferencing and identification of client needs, in order to provide linkage to care/ resources.
  • Manage food inventory for the THRIVES food pantry, including all food shopping for THRIVES at various area markets, as well as any inventory provided through other food/donations sources in the counties serviced by THRIVES.
  • Maintain the pantry in accordance with food safety best practices, including daily monitoring of machine temperatures, as well as utilization of the first-in, first-out inventory model.
  • Completion of program intakes, screenings, and facilitation of client enrollment, including completion of data entry into program database and arrange for required assessments and reassessments with contracted Registered Dietitians in accordance with program requirements.
  • Perform on-going case conferencing and linkage to care/resources for program participants in accordance with program requirements.
  • Attend and participate in monthly department, All Staff and other required meetings.
  • Be familiar with Cornerstone policies and procedures and the Employee Handbook.
  • Maintain confidentiality of all aspects of Cornerstone including, but not limited to, patient confidentiality, financials, and employee relations.
  • Perform other related duties as assigned.

Travel/Time Requirement: Must have a valid, unrestricted driver's license and independent means of transportation for frequent travel to locations throughout the assigned county as well as to the offices of Cornerstone.

Requirements:
  • H.S. diploma and familiarity with social services.
  • Experience in customer/client service(s), organizing and maintaining records and/or inventory, computer literacy. Experience with program promotion and collaboration among stakeholders. Experience with food service and Servsafe concepts is highly desirable.
