Fairview Health Services is seeking a
Nurse Practitioner to work in our
Family Medicine Department to join our primary care practice at
M Health Fairview Clinics – Cottage Grove.
Practice Details:
- 6 provider primary care practice which includes 4 Family Medicine Physicians 1 Nurse Practitioner and 1 Physician Assistant.
- Patient encounters average 18-22 patients per day.
- 32 patient contact hours per week for full time - flexible schedule.
- As a Pioneer Accountable Care Organization (ACO), our physicians use a multidisciplinary team approach in managing clinical care.
- Fully integrated Electronic Medical Record (EPIC).
Income Details:
- Initial 2 year salary guarantee with ability to exceed on an innovative compensation program rewarding production, quality and patient experience.

- Graduate of a school of nursing and accredited Family Nurse Practitioner training program.
- BC/BE through the AANP or ANCC.
- Licensed or ability to obtain APRN license with the Minnesota Board of Nursing.
- Ability to obtain and /or maintain DEA certification for the State of Minnesota.
- Meet threshold criteria/qualifications for Credentialing and Privileges (if required).
