Nexus-FACTS is a community based mental health clinic and part of Nexus Family Healing. In partnership with Hennepin County, Nexus-FACTS has recently launched a first-of-its-kind in Minnesota mobile response service to assist families when a child or youth has escalated behaviors or is in crisis.

Position Summary:

The Responder assists eligible children and their families at times of difficulty and in crisis. Responders support the child/youth and family system in various settings (hospitals, home, school, childcare, etc.) through assessment of needs and providing a plan with access to needed medical, social, and educational services that promote stability, healthy family functioning, and keeping the family together. The Responder is guided and supervised by a clinical supervisor. The position allows for some work from home.

Primary responsibilities include:

  • Believes in, and is inspired by the power of community work
  • Provides quick response time to requests for service
  • Screens for trauma exposure and difficulties to be addressed and demonstrates competence in using screening tool results to enhance outcomes (training provided)
  • Establishes a plan for families based on assessment, with clinical supervisor oversight
  • Identifies resources available and shares knowledge with family and providers on trauma exposure, its impact, and trauma-focused
  • Communicates with youth's family and team regarding crisis support and stabilization
  • Connects family system to resources with a warm hand-off in the determined priority areas
  • Delivers best-practice service throughout community settings as needed
  • Links youth or family to crisis services if required
  • Develops and maintain community contacts and resource building with liaison relationships for easy referrals
  • Provides complete and required documentation, within established timelines relating to planning and stabilization
  • Complies with local office procedures for referrals and service
  • Advocates for the development and maintenance of funding and resources to meet youth and family needs at local, state and national
  • Engages in efforts to strengthen the resilience and protective factors of youth and families impacted
  • Emphasizes continuity of care and collaboration across child-service
  • Understands the potential for secondary traumatic stress and practices self-care strategies to mitigate these effects.
  • Completes paperwork and compile documentation required for governing/accrediting agencies; maintain current files on all
  • Produces informal and formal reports as requested by parents, referral agencies, courts,
  • Participates in individual and group supervision including clinical consultation to review cases and ensure best practices are provided to all youth and
  • Provides timely and thorough information to supervisor regarding case incidence, service delivery struggles and difficult team
  • Utilizes supervision options and directives in service delivery
  • Maintains a schedule that is adjustable and flexible to meet changing work needs and demands of clients
  • Remains current on all company policies, assigned trainings and/or certification
  • Emphasizes and supports cultural connections and support various ways of healing.

Required Education, Licensure, Experience and Skills:

  • Minimum of bachelor’s degree in one of the behavioral sciences, human services or related fields, from an accredited college or university.
  • 1 or more years’ experience in crisis/mobile/residential or community work with children/youth and families
  • Valid driver’s license required. Must meet state regulating agency driving requirements, maintain auto insurance and pass corporate insurance eligibility

Preferred Education and Experience:

  • Experience working within home and/or community settings
  • Preferred experience working in diverse communities and populations

Travel:

Use of personal vehicle for regular local business travel will be required to support the clients and agency business offices or company sponsored events may be required.

Work Environment:

This is a hybrid position requiring in person, remote, and on the road.

This job operates in a professional office environment. This role routinely uses standard office equipment such as desktop and/or laptop computers, photocopiers and smartphones. Must be able to sit in a car, at a desk and computer for extended periods of time; have visual acuity conducive to extensive computer monitor work; able to twist, bend, and stretch in a manner conducive to the execution of daily activities within an office environment. May involve repetitive wrist motion.

Expected Hours of Work:

Days and hours of work are Monday through Friday with rotating weekends, with the expectation to work specific schedules and hours as identified and communicated by management.

While performing the duties of this job, the employee is regularly required to talk and hear. Specific vision abilities required by this job include close vision and ability to adjust focus, and maybe required to lift-up to 50 pounds unassisted and up to 100+ pounds with assistance.
