Medical Services Coordinator/Family Nurse Practitioner Position

A 501(c)(3) nonprofit organization, Child and Family Agency of Southeastern CT (CFA) was founded in 1972 to support and nurture our community’s most vulnerable children and families. We are dedicated to providing compassionate care to our service area. CFA is seeking a Medical Services Coordinator/Family Nurse Practitioner Position to join a team of nurse practitioners passionate about providing high-quality medical care focused on preventative services and health education of students, families, schools, and communities in our School-Based Health Center Program and Outpatient Medical Services. Will be working collaboratively with on-site mental health clinicians to provide a holistic approach to care.

We are looking for candidates interested in a Full-time position, to provide administrative leadership and provide medical services as an APRN to children in our School Based Health Centers and Bridge Clinics (locations are in New London, Groton, Pawcatuck/Stonington, and Waterford, CT). The position will be evenly split administration/clinical practice and is a hybrid/remote position for the administrative functions.

Qualifications: Graduation from accredited school of nursing; Current APRN license as a FNP to work in Connecticut; CT RN license required; Bachelor’s and Master’s degree required. DNP doctoral degree preferred. National certification required. Applicants must be able to perform tasks independently with minimal direct supervision. Position also requires strong organizational skills, the ability to manage multiple priorities, attention to detail and excellent communication skills.

· Perform tasks independently, consulting with supervisory team and peers as needed.

· Provide quantitative and qualitative, thorough review of sessions and summary documentation in electronic medical record database within agency-specified timeframes.

· Participate in general agency (including Risking Connection) and model-specific training

· Assess and support staff in reporting suspected child abuse and neglect within the 12-hour time frame (verbally) and 24-hour time frame (written)

· Provide and document supervision to direct report staff helping to support professional development

· Participate and lead monthly team meetings

· Provide appropriate training opportunities to ensure program competencies

· Complete and review annual evaluations for direct report staff within agency standards

· Act as the liaison between the program staff and the rest of the agency/community to include assisting in crisis management, coordinating care with other providers/funders, being a voice for the program

· Attend regular supervisions and meetings within the department and agency

· Recruit, hire, and train new staff

· Complete disciplinary paperwork and create/maintain action plan with staff as needed

· Coach staff toward performance improvement and professional goals

· Establish and maintain positive relationships with contract holders, including program gatekeepers and DCF, as well as other community providers

· Manage health and sustainability of the program by overseeing timely input of program data and program-specific documentation

· Identify trends in program data and collaborate with Director and contract holders to ensure best practice
