Require :LVN CA STATE LICENSE

Either Full Time: Monday to Friday-5 to 6 hours/day OR Part-time: 1-4 days a week (5-5.5 hours/day)

The LVN shall assist center RN with coordinate, administer of all nursing and personal care services to program participants at ADHC;

The RN shall conduct all nursing QRT assessments, monitor health problems of center participants; , and wellness progress notes daily and weekly.

The LVN shall provide health education and counseling to program participants and their families, conduct health related training to ADHC staff and volunteers, and serve as nursing consultant;

The RN shall refer program participants to appropriate healthcare providers in the community and serve as a liaison with such organizations;

The LVN shall assist key member of the Multidisciplinary Team that provides: Professional Nursing, Social Services, Therapeutic Activities, Physical Therapy, Occupational Therapy, Speech Therapy, Mental Health, and Dietary services to program participants.

The LVN shall assist with any other health or medical relate services needs at the center.

Pay: From $25.00 per hour
