POSITION DESCRIPTION
JOB RESPONSIBILITIES:
Conducts group and individual assessments; provides appropriate case management and follow-up services in an effort to remove barriers to employment and self-sufficiency; monitors participation; provides information and referrals, and performs other related duties as required.
QUALIFICATIONS:
Bachelors degree from an accredited college or university with a major in a human services discipline. Must be verified by transcript.
LICENSURE OR CERTIFICATION REQUIREMENTS:
None required.
ESSENTIAL FUNCTIONS OF THE POSITION:
Contacts clients by phone and/or letter and interviews clients to assess need for services, eligibility, and readiness for applicable programs; completes an assessment of the client's situation to aid in removing barriers to self-sufficiency and employability.
Clarifies and answers questions concerning available programs and services; explains client's rights and responsibilities under the applicable services and benefits; develops an individual self-sufficiency plan and arranges for services
Provides follow-up on all individual plans by monitoring all compliance reporting sources including attendance, participation, enrollment in assignment programs and activities, communications from other LCJFS staff; communications from the client and communications from assignment providers
Ensures compliance with all work requirements by taking required follow-up actions(s); records all assignment and follow-up actions in all required county and state systems; periodically re-determines need and eligibility, determining the effectiveness of the client's plan and making necessary changes; performs all duties in a manner that meets the requirements for participation rates in accordance will all applicable rules and regulations
Completes appropriate forms and reports and maintains case records in accord with agency and ODJFS policy; maintains tracking documents and tools as required; shares case information with appropriate persons; writes and answers correspondence as needed
Accesses basic and case specific information via a variety of resources including computer-based systems and software applications and inputs information; monitors the progress of any case related plans by reviewing available documentation of compliance and progress; takes appropriate action as indicated and ensures that the required documentation is stored in the case file
OTHER DUTIES AND RESPONSIBILITIES:
Performs other duties as required: assists new workers; assists in maintaining the agency workload in the absence of other unit staff; attends meetings, conferences, training sessions; maintains working knowledge of agency policies and procedures, manual regulations and community resources; writes and answers all associated and required correspondence as needed; maintains logs and other tracking documents/tools as appropriate; performs other duties inherent in this position and/or assigned classification.
KNOWLEDGE, SKILLS AND ABILITIES:
Knowledge of: Budgeting, public relations, human relations, office practices and procedures, agency policies and procedures, counseling, interviewing, Ohio Revised Code, Community Organizations operating procedures (websites and written resources).
Skill in: Equipment operation: calculator, computer, word processing, spreadsheets, Internet, email, fax, copy machine.
Ability to: Define problems, collect data, establish facts, and draw valid conclusions, add, subtract, multiply and divide whole numbers, understand meaningful, concise & accurate reports, gather, collate, classify information about data, people, and things, answer routine telephone inquiries from the public.
