Position Overview:
- Conduct initial and ongoing health assessments for our clients as well as provide oversight for home health services throughout New Hampshire.
- Develop, approve and monitor care plans in collaboration with Care Managers and assist in coordinating referrals to community agencies as needed.
- Record data and document appropriately according to department, state and federal requirements while ensuring quality assurance.
- This role will also be responsible for promoting safety awareness and provide clinical training opportunities.
- Registered Nurse license with the New Hampshire Board of Nursing
- Willingness to travel and work flexible hours to meet the needs of the program (mileage reimbursement provided)
- Valid driver’s license and reliable transportation
Hours: Per Diem
