The Home Care On-Call Coordinator is responsible for training and evaluating competencies of the Home Health Aides, Personal Care Attendants, Escort and Companions; as well as providing personal care, assisting with activities of daily living (ADL), meal preparation, bathing, light housekeeping, and/or shopping and errands for clients as outlined by the client’s individual plan of care when needed in the facility. All care/tasks provided are as authorized by the State as acceptable for the level of care in which the Home Health Aide is functioning and must be detailed on the client’s plan of care.


Work Hours: part-time position based on need with on call and weekend responsibility.


Requirements


  • Assist with the development of the Home Health Aide training and competency program. Conducts home care training for new hires and annually as indicated. Completes and tracks Home Health Aide competencies.
  • Expectation: All Home Care employees will complete competency training including hands on simulation in training room or with actual clients.
  • Orients, supervises and evaluates the care provided by the Home Health Aide (HHA) as needed and in accordance with state and federal requirements.
  • Expectation: The Home Care Facility Coordinator will perform and document the orientation and supervision of the HHA as required by state and federal regulations or contracted services. The care provided will be evaluated for compliance with the plan of care.
  • Assists with staffing, scheduling and monitoring client services as directed by calling, emailing, paging or otherwise reaching disciplines necessary to staff the cases and documents the contacts as required by policy.
  • Expectation: Will ensure cases are staffed with appropriate services in a timely manner.
  • Communicates clearly and directly with other staff members and reports any issues or concerns to the Director.
  • Expectation: Communication is ongoing, clear, concise and complete to staff or Director and in a timely manner.
  • Informs Home Care Director of any changes, continuing needs, problems or complaints.
  • Expectation: Home Care On Call Coordinator will notify director of any needs or problems that can adversely affect client care or the goals of the agency.
  • Provides back-up for duties in absence of the Director and participates in on call coverage.
  • Expectation: Will participate in on call coverage and manage complaints and incidents timely and effectively.
  • Assists clients with personal care needs such as bathing, elimination needs, transferring, dressing/undressing, and ambulation together with any household task (e.g. – making bed, changing linens, laundry, light housekeeping, etc.) essential to the client’s health in accordance with the client’s individualized plan of care.
  • Expectation: Clients personal care needs will be met.
  • Plans/prepares nutritious meals and assists feeding clients as directed.
