- Travel to patients' homes and community events
- Complete blood pressure, glucose, and COVID-19 testing as needed
- Learn and apply clinic policies and procedures to COVID-19 and chronic disease complications
- Educate caregivers, family, and patients on ongoing care and lifestyle management options
- Monitor patients and compile reports for physicians
- Provide medical expertise, technical assistance , and coaching to Community Health Worker and the Patient Navigator
- Collect data and/or report on activities that demonstrate progress toward grant objectives to report to Program Director
Position Qualifications:
- Have the ability to work independently with minimal supervision
- Possess the ability to encourage others to seek opportunities for different and innovative approaches to addressing problems and opportunities
- Great written and oral communication skills and be detail oriented
- Must have current driver's license and reliable transportation
Skills & Abilities
Education-LPN, RN, /Associates Degree or higher
- Experience working with Community Resources
- Kansas licensed and certification
