We are looking for an LPN to join our team! We want someone who is outgoing, friendly and ready to help us continue to grow!
Elite Home Care, Day Centers and Transportation is growing quickly and we are in need of an LPN in the Conway/ Myrtle Beach area to assist with home care visits in Conway and the surrounding areas. This is an autonomous role that can really be what you make it! The LPN will oversee the creation of individual plans of care for Clients coming onboard with Elite Home Care. LPN will be responsible for visiting clients and completing the initial sign up within the home, and completing a 30 day visit, and every 90 days thereafter within the home. LPN will ensure sign ups, visits, and care plans are in Compliance with various state funded organizations like Community Long Term Care, Medicaid Waiver programs, and the Department of Veterans Affairs. LPNs will be responsible for recording and documenting their visits within Clear Care and an EVV system run by the state. LPN will be responsible for communicating with Case Managers in regards to visit outcomes, if needed. LPN could also be asked to fill in for Elite Day Centers, only when needed.
The LPN will ensure all required enrollment paperwork is completed with new clients, and communicate with each Office’s Operations Manager in regards to their start date, schedule, and any specialized plans of care they may need. LPNs will not be responsible for scheduling the clients with an appropriate caregiver.
The LPN will review the plans of care, Client Rights and Responsibilities, Advanced Directive Plans with the Client and/or Responsible Party.
The LPN will be responsible for communicating with the Director of Operations and the Director of Compliance when unsure of how to proceed with any issues that may arise.
Job Requirements:
- Experience in clinically working with older adults, those with memory concerns, and adults with disabilities and special needs in either acute or residential/long term care settings or in their homes.
- Must be at least 21 years old.
- Nursing degree required from an accredited college or university.
- Currently licensed in SC to practice nursing as an LPN (required)
- Current CPR certification (required)
