WELLBOUND Certified Home Health Agency is currently seeking FFS Physical Therapist to serve our pediatric and adult special needs population in the home environment. There is paid training, active leadership and an abundance of resources.

Roles/Responsibilities:

• Design and implement individualized treatment addressing skill areas such as fine, gross and visual motor, sensory and emotional regulation, cognition, etc. as they relate to child and adult occupations.
• Complete documentation in a timely manner
• Educate and train HHAs (Home Health Aides) to provide carryover of skills addressed in therapy
• Provide effective and timely communication with families

Qualifications / Requirements:

• NYS License as a Physical Therapist
• Covid-19 Vaccination Required
• Experience using HomeCare HomeBase (HCHB) is a plus but not required.

Job Type: FFS, $85 per visit
