Grace Home Healthcare a HomeCentris Personal Care company is seeking a weekend overnight Licensed Practical Nurse (LPN) for our Personal Care division in our Fairfax, Virginia office. The LPN will help our clients remain in the setting they most desire to be in, their homes.

Duties:
- Assesses patient’s condition, initiates plan of care developed by the Case Manager, re-evaluates and updates as necessary.
- Follows the five (5) medication rights and reduces the potential for medication errors.
- Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
- Other duties as assigned.
Requirements:
- LPN license on VA Board of Nursing.
- Two years of experience as an LPN.
- Homecare experience preferred.
- Weekend and overnight availability.
- Ability to work independently.
- 12-hour shifts.
