What Will you be Tasked With?
  • Coordinates and oversees all direct and indirect patient services provided by organizational personnel.
  • Supervises clinical staff effectively and completes performance actions as indicated and documents appropriately.
  • Interprets personnel policies to office staff members, reports all personnel matters requiring action to the Administrator as appropriate, and promotes staff recruitment, retention, and employment supervision.
  • Participates in establishing community contacts and developing relationships to promote community education and resources to coordinate services and plans and promotes program development.
  • Monitors utilization of services and ensures services are provided in accordance with applicable regulations and third party payer guidelines.
  • Identifies and reports any unusual occurrence, accidents or adverse effects of medical devices and documents appropriately. Assists with follow up review and documentation.
  • Serves on Professional Advisory Committee and directs quality/performance improvement monitoring and activities.
  • Provides leadership in the assessment of patients and their families and in the development and modification of plans of care as necessary.
  • Takes referrals and makes initial assessment visits, or ensures that assessment visits are made, and makes intermittent and on-site supervisory home visits when directed, or necessary, and organizes family resources for effective patient care.
  • Demonstrates leadership in promoting and maintaining standards for quality care and supervises health care practices to assure that the standards of care are maintained.
  • Ensures the completion of competency and performance evaluations of staff and documentation of same as required.
  • Responsible for the effective, efficient assignment, scheduling and supervision of clinical staff and on-call coverage and maintains records of case assignments and management forms as directed.
  • Organizes and directs patient care conferences, inservices and works effectively with Health Care Providers and other members of the home health team to promote coordination of patient services, positive patient outcomes and effective documentation of same.
  • Reviews medical records routinely and conducts clinical record reviews to supervise the quality and timeliness of care and documentation. Investigates adverse events to identify opportunities to improve care and follows-up as indicated.
  • Consults with the Administrator as necessary and informs same when staff recruitment, assistance or supplies as needed to assure the safe and effective delivery of care.
  • Ensures office is compliant with applicable Elevate Home Health, state, federal, local and Medicare regulations.
  • Provides assistance to the Administrator in coordination and management of all services rendered to patients and families by direct and contract staff.
  • Participates in the development and revision of policies and procedures affecting services.
  • In the absence of the Executive Director/Administrator the DPCS will become the acting Executive Director/Administrator and will be vested with the authority to act in behalf of the Executive Director/Administrator.


Environmental Conditions and Physical Requirements
  • Driving in a motor vehicle, standing, sitting, walking, bending, reaching and stretching.

Minimum Requirements:
  • A registered nurse with a baccalaureate or higher degree in nursing or other health related field with three years of experience within the last five years in a home health agency, at least one year of which was in a supervisory or administrative capacity OR (2) a registered nurse with four years experience within the last five years in a home health agency and at least one year of which was in a supervisory or administrative capacity.
  • Excellent interpersonal, verbal and written communication skills and ability to recognize the needs and concerns of people and maintain constructive working relationships with them.
  • Must have a vehicle, driver’s license and appropriate automobile insurance coverage.
  • Ability to make home visits and provide skilled nursing assessments and care in the home environment.
