Position Summary:
  • Administering appropriate first and second level screening tools for all patients ages 12 and older
  • Facilitating patient access to treatment, including access to CCHC, community and other recovery resources based on screening outcomes and patient agreement.
  • Collecting and entering baseline GPRA data for patients with alcohol or other drug (AOD) use disorders into REDCap
  • Collecting and entering baseline screening data for patients in the EHR
  • Tracking patients’ substance use disorder (SUD) treatment progress and assisting the evaluation team in six-month patient follow-up for GPRA data collections.
  • Promoting diverse cultural health beliefs and practices, patient referred languages, health literacy and other communitarian needed for the population of service.
  • Assisting in the implementation of all SBIRT, data collection, and evaluation related activities including monitoring and implementation
  • Responsibilities Include but are not limited to:
  • Screening/Assessment and Care Coordination
  • Responsible for screening services to identify the presence or absence of alcohol or other drug (AOD) use where data is collected on a patient in order to make an initial determination if a problem exists and/or to determine if referrals for emergent needs are warranted.
  • Responsible for assessment which includes collecting information to ascertain the degree and severity of alcohol and other drug use, as well as to identify the social, physical, and psychological effects of that use.
  • Responsible for placement, which includes determining the appropriate level of care, type of service, length of stay and the most appropriate facility for a patient
  • Responsible for determination of eligibility for treatment services. This is the process of determining, along with the patient, what type of service is assessed as clinically appropriate.
  • Interface with CCHC primary care providers, the Medication Assisted Treatment (MAT) Care Team, and behavioral health providers.
  • Responsible for providing case coordination to ensure that the patient’s holistic needs are addressed
  • Will work in cooperation with the social service agencies and community resources in planning to meet the needs of patients and assisting patients with utilizing these resources.
  • Performs additional SBIRT or screening activities in accordance with requests from Access Community Health and CCHC.
  • General Duties
  • Ensure that all contract scopes and criteria are met or exceeded
  • Obtain & maintain certifications/licensure/trainings as needed
  • Complete documentation and data entry as needed to assure optimal patient care and program reporting
  • Participate in evaluating outcomes at the individual level with each patient and at the same time participate in agency-wide evaluative and quality improvement efforts
  • Engage in community planning groups and/or meetings to support the social and health care needs
  • Serve as part of the patient’s care team and support health center operations and attainment of organizational metrics
  • Establish effective and respectful relationships with patients, families, professionals, payers, and other relevant parties
  • Assist in developing/maintaining community referral relationships and effectively connecting patients and families to community resources
  • Perform other related tasks as needed or assigned
Minimal Qualifications/Experience/Skills:
  • Current Certified Alcohol and Drug Counselor (CADC)
  • High School Diploma with 2 years of experience in Case Management, Social Service Programs or Community Health.
  • Ability to establish and maintain effective working relationships with colleagues, providers, patients, collaterals, other stakeholders, and the general public
  • Ability to utilize technology to complete job duties.
  • Ability to make sound decisions in accordance with SBIRT contractual standards, CCHC policies and procedures.
  • Ability to organize and prioritize a variety of demands on time.
  • Proficient knowledge of current alcohol, tobacco and other drug prevention, intervention, treatment practices and recovery-oriented system of care philosophies we well as mental illness.
  • Ability to speak publicly in an effective manner.
  • Valid driver's license and current auto insurance.
  • COVID-19 Vaccination is a condition of employment.
