What you will do?

The Community Support Specialist/Education Liaison is a full time position developed in partnership between Four Oaks and Fairfield Community School District. This position is integrated into the District’s Learning Supports Team and provides support for school staff using Restorative Practices and a Trauma Informed approach. The Community Support Specialist/Education Liaison is designed to bridge the gap between a student’s home and school life by providing ongoing resources, referrals, case management, and skill building services to the student/family, as well as creating a supportive school environment for the student/family’s needs. This position is hired and supervised by Four Oaks, but embedded into the daily operations of the school district. Specific responsibilities include:

CSS Primary Duties

• Attend district Learning Supports, Tier 2/3 meetings, and counselor meetings as designated by district leadership.
• Offer consultation to the team about services/supports available to kids/families who may be brought to the team due to mental health or trauma concerns
• Student/Family engagement through building relationships and completing assessments using Four Oaks model to determine family needs and appropriate supportive services
• Make referrals and help family to engage with community based providers (such as Mental Health care, basic needs assistance, etc)
• Maintain a caseload of approximately 25 students at any given time as referred by the district, and provide ongoing case management and support for clients
• Attend home visits, appointments, and regular meetings with students/ families as needed to ensure participation in ongoing supportive services
• Provide ongoing data tracking and progress monitoring to the district staff on the student/families needs and supports
• Be the single point of contact between the family and the school

CSS Secondary Duties

• Help provide individual and small group interventions via Tier 2 supports for identified students
• Facilitate Professional Learning around specific topics to the district staff
• Offer classroom supports and feedback to students and staff

Four Oaks Supervision duties

• Attend weekly supervision meetings, all team meetings, and ensure accurate and timely documentation requirements

What you need:

You will need to possess a Bachelor's degree in a social services, Education or related field and 2 years of prior experience or a Master’s degree and 1 year of experience. Additional requirements include

• Effective relationship building skills in the creation of new partnerships
• Self-motivated and goal-oriented/driven to achieve performance goals
• Strong oral presentation and public speaking skills.
• Basic computer skills in a MS environment
