Job Title: Community Partnerships Manager

Department: Health Equity

Supervisor: Director of Health Equity

Location: This hybrid position requires in-person (activities based in DC and Maryland) and remote responsibilities as needed.

Classification: Exempt/Hourly position


Synopsis:

Promoting the organization's mission and vision, the Community Partnerships Manager is responsible for administering and implementing the organization's Community Partnerships Program yearly work plan and cross-sectoral network, including oversight of crucial legal services and language access partners. In addition, this role is responsible for developing key performance indicators and dissemination strategies relating to La Clinica's participation in diverse coalitions, promoting community engagement and involvement in advocacy efforts, and mapping social needs. The incumbent must foster relationships and organize systems that adhere to company policies and established budgets.


Required Education and Experience:

  • Master's degree in the health field, related discipline, or an equivalent four-year combination of education and experience from which comparable knowledge and skills may be obtained.
  • Bilingual, Spanish English / bi-cultural.
  • Excellent verbal and writing skills are required.
  • Knowledge of local health policy environment in Washington DC and Prince George's County.
  • Commitment to advance Latino immigrant health, reduce immigrant health disparities and health equity issues required.
  • Strong quantitative and analytical skills and ability to manage, interpret and data sets.
  • Demonstrated understanding of digital platforms, including live streaming and graphic design platforms
  • Demonstrated ability in program and project management and evaluation practices, including assessing outcomes and impact.
  • Computer proficiency in Microsoft Word, Excel, and PowerPoint and a willingness to develop additional skills as needed, required.


Preferred Skills/Abilities/Certifications/Licenses:

  • Certification in participatory/community engagement methodologies.

Duties and Responsibilities:

  • Conduct ongoing assessments of community health needs based on program data, health agency reporting, and community appraisals with target populations and disseminate findings.
  • Convene and co-facilitate community workshops and community assessments to map significant health issues affecting the Latino Immigrant Community.
  • Develop and update yearly program work plan for Medical Legal Partners.
  • Oversee timely entry of Simultaneous Interpreter requests and manage a cadre of interpreter's invoices, contracts, and protocols for in-person events.
  • Develop educational and promotional materials.
  • Convene, co-facilitate, and participate in crucial stakeholder groups/coalitions to develop and implement strategies to address inequities in health insurance access for Latinos and other immigrants and advance greater understanding, regionally, of the role of immigration as a social determinant of health. Provide written reports as required.
  • Ensure regular and timely updates of La Clinica's Health Equity and Community Partnerships Hub.
  • Work with the Director of Health Equity to develop recommendations on standardized systems to collect the social determinants of health.
  • Organize, evaluate, and continually report on La Clínica's Advocacy Committee work plan, including:
    • Convene and co-facilitate LCDP's Advocacy Committee monthly meetings.
    • Track yearly progress on LCDP's advocacy agenda and provide written reports of activities as required.
    • Meet and provide support as necessary with individual committee members to support the implementation of the yearly agenda.
    • Organize and track LCDP's participation in external coalitions and update regularly.
    • Ensure staff and community participation in yearly budget hearings by providing testimonies in both English and Spanish for City Councils.
  • Provide policy recommendations to Senior Leadership.
  • Recruit and train a cadre of Health Promoters to implement community mobilization and awareness campaigns.
  • Ensure LCDP's patient advisory committee regularly meets and develops recommendations on patient-centeredness and community engagement, including involvement in significant advocacy efforts.
  • Coordinate training and provide technical assistance to staff, managers, directors, and external audiences on key immigrant health equity issues.
  • Organize logistics for community forums and coordinate LCDP's "Dialogues for Action, and La Clinica Talks" including securing speakers; utilize appropriate methodologies for event implementation; ensure right technology is used for event; and provide written follow-ups of meetings, and briefings, workshops and events.
  • Engage community leaders, community members, policymakers, community-based organizations, health centers, private and public insurance organizations and to advance the organization's health access and health equity goals.
  • Coordinate LCDP's participation in projects and coalitions addressing health equity for Latinos and other immigrants in suburban Maryland.
  • Ensure completion of LCDP deliverables under local and federal contracts.
  • Provide monthly written reports on progress meeting work plan.
  • Administer assigned program budget, ensuring expenditures are according to the approved budget.
  • Supervise program coordinators and staff, including training, monitoring, records review, scheduling, timesheets, and performance evaluations.
  • Manage supplies for program activities, paperwork, client incentives, and program supplies required to run the program.
  • Ensure timely entry of encounter data into required program databases.
  • Support administration of assigned program's expenditure budget including payments reporting, as planned by supervisor, ensuring expenditures according to approved budget.
  • Attend program, department, and all staff meetings including all mandatory La Clinica activities.
  • Meet with supervisor as required to coordinate implementation of programs and services.
  • Represent La Clinica by attending conferences, meetings, coalitions, and community planning groups meetings, as designated by supervisor.
  • Attend required trainings to increase/maintain skills.
  • Follow La Clinica's Code of Conduct, and all other personnel Policies and Procedures.
  • Perform other related duties as assigned.

  • Prolonged periods of sitting at a desk and working on a computer.
  • Ability to work weekends and evenings as needed.
  • Able to perform duties in different La Clinica's locations as required.
  • Able to perform duties and responsibilities remotely when needed.
  • Ability to lift/push at least 30lbs during community events.
