Chart Analyst
Full-Time

Overview:

Support a high efficiency team within the HIM department that helps insure compliance at all times for all patient records and medical information. The Chart Analyst will analyze all records for completion for discharged patients to ensure compliance with hospital policy and regulatory requirements. Will monitor and complete all work queues for chart deficiencies and errors. The Chart Analyst will also monitor the Right Fax queue for information from outside facilities and will index that information appropriately into Perceptive and EPIC.

HIM Deficiency Analysts is responsible for analyzing medical records for completeness and compliance in accordance with federal and state regulatory guidelines, Joint Commission standards, Medical Staff By-Laws/policies and procedures, and HIM department policies and procedures. This includes reviewing medical records for completeness and assigning deficiencies to appropriate clinical staff, monitoring deficiency analysis work queues and resolving related problems. The Chart Analyst is a resource for continuum of care services to hospital staff, medical staff and patients.

ROLE RESPONSIBILITIES
  • Analyze medical records for completeness and compliance.
  • Identify canceled encounters and notify appropriate HIM staff.
  • Monitor work queues and perform problem resolution/corrective action where needed.
  • Reassign dictation deficiencies that are marked as "complete" by the physician.
  • Assist physicians in chart completion by clarifying the deficiency or explaining the chart completion process in Epic.
  • Assign documentation deficiencies to the correct clinical staff.
  • Collaborate with the release of information team to ensure request documentation is complete prior to release.
  • Perform documentation deficiency analysis for inpatient discharges according to established State and Federal Guidelines, Joint Commission Accreditation Standards. Medical Staff Rules and Regulations and Department Policies.
  • Ensures completion of required medical record documentation.
  • Maintains required records, reports, and files as directed.
Licenses and Certifications
RHIA / RHIT Preferred

Qualifications:
Education and Experience:

High School Diploma, GED, or a higher level of education that would require the completion of high school, is required. Associates degree preferred.
1-3 years required experience in Deficiency Analysis or Chart Analysis

KNOWLEDGE OF:

Knowledge of Windows Operating System, Microsoft Office products, Electronic Health Records, Document Imaging Systems, and office equipment. Knowledge of medical terminology anatomy and physiology, medical procedures, medications, and laboratory test. Knowledge / Basic Understanding of State and Federal Guidelines, Joint Commission Accreditation Standards. Medical Staff Rules and Regulations and Department Policies.

SKILLS:
Active listening, Attention to detail, Problem solving, Critical Thinking, and prioritization. organization and time management.

ABILITY TO:
Effectively uses computer software and technology as required by the organization including Microsoft Word and Excel. Uses fax, copier, phone system and other required tools. Locates the records for the patient in multiple systems as required. Ability to work independently as well as in a team environment. Ability to communicate effectively both verbally and written

WORK ENVIRONMENT:
Fast paced noisy environment.

PHYSICAL REQUIREMENTS:

The position requires working predominantly in a sedentary manner there are times that will require some walking, bending, reaching, stooping, lifting, carrying and pushing of supplies and medical records.
