Job Duties

$3,000 sign-on bonus and quarterly retention bonuses through June 2023 for those who are new to State service!!

Southern Virginia Mental Health Institute, a 72-bed Joint Commission accredited State Mental Health Facility, is seeking an energetic, clinically skilled, organized individual to provide a range of casework activities related to a comprehensive needs assessment involving each patient’s discharge planning process for psychiatric patients in an inpatient setting. This position will serve as a Case Manager, providing an array of services including Case Management, psychosocial assessments, and social work services on the unit and may serve on department and facility committees. The Case Manager will coordinate the discharge planning needs of assigned patients, with treatment team and CSB staff.

Minimum Qualifications

A Bachelor’s degree in Social Work (BSW) from an accredited school of social work with case work/group work experience.

The following Knowledge, Skills and Abilities are required of this position:
Fundamental knowledge of the principles, practices, services and techniques of case management.
Demonstrated ability and skills to assist in coordinating resources for patients.
Ability to perform duties at a fast pace with frequent change.
Ability to interact with other professionals from various disciplines.
Ability to maintain safety precautions.
Ability to form a working relationship with patients and families; determine and select services; and communicate effectively, verbally and in writing.
Must have a valid driver’s license.
Must be able to work flexible schedule.
Must be able to provide thorough documentation in an electronic health record (EHR).

Preferred Qualifications:

Advanced knowledge of mental illnesses and dual disorders.
