Job Title: Case Manager - Critical Need (13 weeks)

Address: Waipahu, HI - 96797

Shift: Days (8*5) 8:00 AM to 5:00 PM

Rate: $60-$65/hr. on W2 (40 Guaranteed hrs.)

Description:

• Experience as a case manager.
• Experience working with clients to develop a treatment plan.
• Sound knowledge of legislation relevant to cases.
• Background in health care, social work, mental health, disability care, substance abuse recovery, geriatric care, and long-term care.
• Active BLS from AHA.
• Valid HI State License.
• Covid vaccine certificate.

Responsibilities:

• Health Coordinators (nurses & social workers) assess the needs of eligible Medicaid members then authorize and arrange the services & supplies as appropriate.
• Provides care management (CM) and care coordination to both QI community and institutional members.
• Completes the Face to Face Health Functional Assessment and Service plan.
• Care management is provided across the continuum of health care needs - the community, hospital, skilled nursing facility or institution.

Job Type: Contract

Pay: $60.00 - $65.00 per hour
