Who We Are
St. Joseph's/Candler is rich in history and heritage. Its anchor institutions, St. Joseph's Hospital and Candler Hospital, are two of the oldest continuously operating hospitals in the nation. As such, both have been a part of the Savannah landscape and culture for a combined history of more than 350 years. In 1997, the two legendary hospitals and their various entities developed a joint operating agreement, creating the largest health system in Southeast Georgia and the South Carolina Low Country.

What You’ll Do

• Serves as both a functioning member of the LCRP patient care team as well as the Lead Social Worker for LCRP.
• Coordinator shall be knowledgeable about the psychosocial impact of the disease of cancer on individuals and families as it relates to their age and stage of development.
• Provides proactive psychological and social support services to patients, families and significant others.
• Coordinator completes comprehensive patient assessments and addresses identified needs with appropriate interventions.
• Functions as a resource and educator for social services.
• Coordinator develops and oversees an ongoing dynamic and organized patient/family support program that addresses both psychosocial and financial needs of patients dealing with cancer care.
• Coordinator must have clear written and verbal communication skills and be compliant with all laws and regulations.
• Demonstrates ongoing leadership within the hospital, community and professional organizations.

Our Requirements

• Bachelor’s degree – Required
• Master’s degree – preferred
• 5-7 years social work experience – required
• 3-5 years hospital social work and case management - preferred
