The Care Coordinator position is a great opportunity for someone with a passion for helping and assisting the elderly, disabled and veteran population.

The responsibilities of our Care Coordinators are:

*Initiate, build, and maintain relationships with our clients and their family members
*Create the best plan of care for your clients during in home assessments.

*Interview, hire and manage caregivers

*Go out in the community and build relationships with key individuals serving the same group of individuals.

*Initiate, grow, and maintain relationships with the team of caregivers delivering care to your assigned clients.

*Document key activities to ensure compliance with Illinois Department of Public Health regulations, and to ensure key information is available to the teams supporting

The ideal candidate:

Must have strong computer skills and able to write, send and communicate professionally through emails.

Has excellent critical thinking skills, decisive judgement, and the ability to work productively with minimal supervision. He/She can maintain composure and positive thinking during crisis situations. He/She is an excellent communicator, and can communicate effectively orally and written with individuals and in small groups. He/She can accurate documentation. Must have pretty good computer skills and a be a quick learner.

Has a driver’s license and dedicated use of a vehicle to drive to clients and our office. Also, MUST be FULLY VACCINATED including Booster.

Have knowledge of the NWI area. Able to balance field/office days.

Experience preferred.

Job Type: Full-time

Pay: $18.72 - $20.06 per hour
