Care Coordinator
Job description
Seeking a highly organized professional with excellent communication skills, with at least 2 years of
experience, seeking a full-time position as coordinator of home care services at two Independent Living
communities for a well-established Home Care Agency.
Job duties include daily coordination of services for clients at the communities, sharing 24/7 on-call
duties with the company scheduler, entering employee timesheets into company software for payroll
and billing purposes, updating client service requests and providing input to nurses to update care plans
as client needs and requests change, miscellaneous filing, answering phones with a friendly attitude and
checking email and phone messages on a routine basis.
