We are a small locally owned and operated hospice agency in Sparta, IL in Rural Southern Illinois. We are actively looking for a Medical Director to oversee our hospice agency and the care for our patients.

Requirements: Must have a valid Illinois Medical License; Doctor of Medicine/Doctor of Osteopathic Medicine.
Must be available by phone

Some of the job responsibilities:

· Palliation and management of terminal illness and conditions related to the terminal illness

· Reviewing the clinical information of the referring physician to document basic disease process; drug regimen; and assessment of the patient’s health and prognosis at time of admission

· Perform an admission history and physical for each patient who has no other physician

· Assist in developing plan of care for each family/patient with the coordination of the patient’s attending physician

· Medical care interventions targeting unmet general medical needs

· Hospice Pre-election evaluation and counseling services, including patient consultation

· Compliments attending physician care by collaborating and cooperating with the attending physician and the interdisciplinary team

· Supervises all hospice physician employees and contract hospice physicians

· Participation as a medical resource person in interdisciplinary team meetings to review active cases and in the establishments and continual development of written plans of care at specified intervals as dictated by the agency

· Facilitate clarification of patient and family goals for care. This includes attending and participating in patient/family care conferences, when requested

· Review the active medical care and palliative care in patients home and any inpatient setting in which hospice services are provided

· Education of staff, coworkers, colleagues and attending physicians on hospice admission criteria and Palliative care

· Assures overall continuity of the hospice medical services

· Assures that the patient receives appropriate measures to control uncomfortable symptoms (ensure pain and symptom management and control)
