Horizon Health Care is seeking a PRN Advanced Practice Provider for our North West Region. Under the direct supervision of the Chief Medical Officer, the Advanced Practice Provider (APP) provides direct patient care for all patients. The organization is looking for a highly professional individual with strong communication skills and committed to coordinating all aspects of primary care for the patients served.
Successful candidates for the APP should be passionate about providing high-quality, patient centered care. Horizon Health Care offers a team-oriented work environment focused on meeting the needs of individual patients while supporting and contributing to the health and wellness of our communities.
This position will provide coverage to Horizon Health Care Clinics in Yankton, Elk Point, and Alcester.
