MINIMUM QUALIFICATIONS:

  • Associates AND two years' experience, or equivalent education plus experience working in a social service/health care agency providing client related services to diverse populations (e.g., LGBTQ, People Who Use Drugs, BIPOC, individuals with mental health needs, etc.).
  • A valid NYS driver's license and access to a reliable and insured vehicle is required.
  • Excellent verbal communication and good documentation skills are required.
  • Proof of COVID-19 vaccination is required.
  • Experience delivering stigma-free, non-judgmental and culturally competent services.
  • Sensitivity to HIV/AIDS, chronic illness, LGTBQIA+ issues and a strong commitment to the mission, vision, and values of ACR Health are essential.

PRIMARY RESPONSIBILITIES:

  • Incorporate best practices regarding confidentiality into all job duties and communications in accordance with Article 27-F, 42 CFR, and HIPAA. Adhere to ACR Health policies and procedures and other applicable regulations.?Protect agency data in accordance with required protocols.?Observe all applicable confidentiality laws and regulatory requirements.
  • Provide the direct provision of HIV/STD/Hepatitis prevention interventions to the target population(s) outlined in assigned program(s) work plan(s).
  • Conduct engagement of individuals through InReach (to SEP and HCV Navigation programming) and outreach activities (hot spot and peer-supported) who are eligible for MOUD screening and linkage activities.
  • Complete screening and intake activities for individuals eligible for connection to low-threshold clinical services for MOUD. Conduct linkage and navigation services in order to connect eligible individuals to telehealth buprenorphine induction and maintenance care.
  • Provide supervision, oversight, training, and task assignment to the Harm Reduction Specialist ? Oneida, as well as departmental volunteers, interns or peers.
  • Maintain accurate client and service records in accordance with the Syringe Exchange department?s policies and procedures. Complete documentation within required timeframes.
  • Complete accurate monthly, quarterly and/or annual report(s) within agency and departmental guidelines. Assist with completing funding applications as requested.
  • Be present in Utica, as well as surrounding communities in Oneida County, to network with providers, providing outreach services in the community and facilitating educational lectures/presentations.
  • Identify and develop networking relationships with community-based organizations.
  • Assist with the general work of the Harm Reduction department, including special projects and general agency programs, outreach/tabling, client recruitment and engagement, screening and needs assessments, referrals and linkage activities, etc.
  • Participate in appropriate and applicable training and professional development opportunities for a minimum of 24 hours of curriculum-based training annually.
  • Complete all required initial and ongoing agency training offerings, as well as job specific training curricula, initiative required competencies, and provider conferences. Provide documentation and attestation of such as per policy.
  • Participate as a culturally competent and culturally sensitive member of the department and agency, which includes having the capacity to function effectively as an individual in the context of various cultural beliefs, behaviors, and the needs presented by agency clients.
  • Maintain confidentiality of all agency clients, as well as other appropriate organizational components, or proprietary programmatic information.
  • Maintain all agency and program property/equipment in good standards. Cooperate with the IT department for routine security checks and maintenance of equipment(s).
  • Perform daily checks of agency vehicle, as relevant, ensuring its safety and sufficient fuel.
  • Work in a cooperative, collegial style with all other organizational personnel. Conduct oneself in a professional manner as an agency employee and representative.
  • Perform other duties as assigned by the Manager and/or Director of Harm Reduction Services, Chief Officers and/or other designated agency staff members.

Salary Range: $21.50 - $23.10/hr OR $39,130 - $42,315 annually (35-hour work week).

Non-exempt. Thirty five hour (35.00) week; evenings and weekends expected for meetings, workshops, and seminars.
