We offer traditional nursing services such as therapy, hospice care, long-term care, and skilled nursing. Our rehabilitative therapy services include physical therapy, occupational therapy, and speech therapy, along with therapeutic recreation.

Requirements for Employment
Bachelors Degree in Social Work or Human Services required
Experience working with elderly and/or long term health care preferred
Strong written and verbal communication skills, ability to problem solve and handle stressful situations
COVID-19 Vaccination or approved medical or religious exemption
Pass a Criminal Background Check, Pre-employment Physical and Drug Screen
Job Summary
As a part of the Social Work team this position is responsible for assisting elders and their family with their transition to the facility, complete MDS, elder assessments, and updating elder care plans. Process referrals, home evaluations and admission paperwork. Identify the social, emotional, and financial needs of the elders and arrange appropriate services. Provide counseling and problem solving with the elder and their family. Participate and lead various interdisciplinary meetings, maintain records, charts and progress notes in accordance with State and Federal regulations. Assist with discharge planning and referrals. Deal tactfully and maintain positive relationships with co-workers, elders, visitors, volunteers, and regulators. Maintain a clean and organized work space.
